"You've been living underground, eating from a can" is probably symbolic for his running away from stuff, and the can representing his means of survival. They are both kids- he is a young teenager and she is probably younger. Towards the end of the song, he talks about "the spirits moving them". Despite some people pointing out possible demonic interaction here, it's probably just alcohol that he snatched from his dad's liquor cabinet.
